Dan Salas had obsessed over the average size or his penis for most of his adult life. He avoided joining the Marines for fear that he wouldn't measure up to other guys in the shower, and he stayed

Ironically, the operation he had sacrificed so much for cost him inches, as well - his penis shrank from five-and-a-half to four-and-a-half inches. So he consulted with a medical doctor who informed him that for several months after his surgery, his penis would actually retract and that excessive scar tissue could permanently shorten it. After the doctor introduced him to a device that would stretch his penis back to size, Salas began the long-term process of trying to regain his phallic length.
Fixations on penis size and doubts about size-related sexual performance have become universal male preoccupations. "For some reason, men equate size with manhood," says psychotherapist Dr. Ana Robles. "Even men of adequate size often feel there's something wrong, that they're small. But size definitely matters, because it builds self-confidence. A man feels more like a man, more sexual prowess, with a larger penis."
During the past several years, Dr. Richard Wineland, M.D., an urologist with the Anaheim Urologic Medical Group in Anaheim, Calif., says he's seen an increase in the number of men seeking some form of penis enhancement. "I would say we get four inquiries a week from men interested in the surgery and other options," says Wineland.
Whether one's goal is to increase penis length or girth, there are a number of effective methods. But the matter should never be approached without extensive research, along with careful consideration of the hazards and expenses.
While surgery is the most immediate solution to size concerns, it's expensive and has the highest risks of complications. Also, the two most common penile surgeries don't change the size or structure of the penis; they only make it appear longer by either cutting the abdominal ligaments that anchor it to the body of extruding the part of the penis that usually runs inside the abdomen (mini-abdominoplasty). Then, to prevent shrinkage during the healing process, patients must hang weights from their penis on a daily basis for up to a few months following the procedure. In order to increase girth, another operation involves the transfer of skin from other parts of the body, or tissue from cadavers, to the penis. Any of these surgeries can cost between $7-12,000, and carry the risk of infections and other complications.
Sadly, one of the most popular solutions is the least effective. The billion-dollar penile-enlargement industry is being fueled, in part, by companies hawking pills that are supposed to make a man's penis larger. But experts say there's no evidence so far that any of these products work. The idea of making a decision about one's manhood based on claims — neither scientifically nor medically validated - by corporations that are trying to sell penis growth pills is downright scary.
"There are a variety of pills and creams that can give a man short-term - less than one day - enlargement of the penis," says pharmacist Dr. Dana Nelson. "But the duration is 4-6 hours only, and all medications have the potential for negative side effects, especially with people who have high blood pressure or diabetes."
On the other hand, one of the most successful (yet limited) methods for increasing penis length is done by carefully stretching tissue in a controlled manner over long periods of time. Doubtless there are some insane solutions offered on the Internet, so one should seek only medically approved techniques. Negative pressure vacuums, which were originally created to treat erectile dysfunction, can temporarily produce a degree of length and girth enlargement. But there are toy vacuum systems of poor quality on the market, so patients should seek only medically approved systems.
As the publics fascination over penis size continues to escalate, so will the variety of enhancement procedures and supplements. But considering the far-reaching, life-changing prospects that come with trying to build a bigger penis, making sure you do an adequate amount of homework - as well as a great deal of soul-searching - cannot be emphasized enough.
